Patient registration

If you would like to join Victoria Medical Centre you must live within our practice catchment area. This covers most of the SW1 and some of the W1 postal districts but please check with reception.

You will need to fill in a Registration form and a new patient questionnaire. These are available from reception in a new patient pack or can be downloaded by clicking the links below.

Completed forms can be handed in to the practice or sent through the post together with two forms of ID confirming your address.

New patient registrations are done Tuesday - Friday in the mornings only. You must book an appointment in advance to register.

Proof of address and photographic ID are required to complete your registration.

To complete the formalities please bring into the practice 2 forms of ID i.e. utility bills or bank statements.
