Patient registration

If you would like to join Victoria Medical Centre you must live within our practice catchment area. This covers most of the SW1 and some of the W1 postal districts but please check with reception.

You will need to fill in a Registration form and a new patient questionnaire. These are available from reception in a new patient pack or can be downloaded by clicking the links below.

Completed forms should be handed in to the practice together with two forms of ID. Proof of address and photographic ID are required to complete your registration

Registration appointments are only bookable once we have seen the completed documents with the correct proof of ID.

 

Registration times are Monday - Friday 2pm - 3pm, Saturdays between 9am and 11am.

To complete the formalities please bring into the practice 2 forms of ID as stated above.
